EXTRA TICKETS FOR SOCIAL FUNCTIONS
Full-time registration includes one
ticket for the Welcome Reception and
Conference Dinner. Extra Conference
Dinner tickets for additional guests
must be ordered on the
registration form at a cost of
A$95.00 per person.
BOOKING
SEATS FOR SPECIAL PROGRAM SESSIONS
When you register you can book
seats for the Meet The Professor
and Women's Breakfast sessions.
Concurrent sessions are held at
7:00am on Monday 10th and on Tuesday
11th September 2007 - see
the program for session details.
The cost for each session is A$30.
Sessions are limited to 30 people.
PAYMENT OF FEES
All payments must be in Australian
Dollars.
CREDIT CARDS
Mastercard or Visa accepted
(Diners and AMEX NOT
accepted).
Online payments will be
processed immediately via a live
secure transaction gateway.
OTHER METHODS OF PAYMENT
Payment can be made by international
bank draft drawn on an Australian
bank. Australian delegates may pay
by cheque. All bank drafts and
cheques should be made payable to
“ANZ Bone & Mineral Society”
ATTENDANCE CERTIFICATES
Letters of verification of
attendance will be available on
request at the registration desk.
PRIVACY
Personal information, as defined
under the national privacy
legislation will be treated in
accordance with the National Privacy
Principles and only shared with
related or third parties in
accordance with those principles.
Please complete the section of the
registration form relating to
privacy if you do not wish your
personal information to be included
in the published Participant List to
be distributed at the meeting.
CANCELLATION AND REFUNDS
A refund of registration fees, less
an administration fee of $A100.00,
will be made on cancellations
received in writing before 1 August
2007. After that date, refunds will
be made at the discretion of the
organisers.
LIABILITY
In case of industrial disruption,
the ANZBMS accept no responsibility
for loss of monies incurred by
delegates. If, for reasons beyond
the control of the organisers, the
conference is cancelled,
registration fees will be refunded
after deduction of expenses already
incurred.
ACKNOWLEDGMENT OF REGISTRATION AND
RECEIPT
All registrations will be
acknowledged by email and will be
accompanied by a tax invoice and
receipt for payments received.

and notify the Conference
Secretariat immediately if any
changes are required.
